Note:  All of the documents and forms are saved in two file formats:  Microsoft Word and Adobe Acrobat.   Which format you choose depends on what software you have on your computer.   If you have Microsoft Office (and Word) you should choose the "Word" format.   If you do not use Word (or am having trouble opening the form with Word), then you should download the "Acrobat" format.   

If you choose the "Acrobat" format, you will need the free Adobe Acrobat Reader program to view the documents.   If you do not have this program installed, or are not sure, please click here  to download it from Adobe's website (http://www.adobe.com).  Follow the website's instructions to download the right version for your computer and for details on how to install the Reader software.
